How GPs refer mental health services

There are two ways a general practitioner can refer their patient to a South Western Sydney PHN funded mental health service. 

 

Complete the online SWSPHN Mental Health Referral Form

 

Use the SWSPHN Mental Health Central Intake template within your practice software 

For assistance with implementing the SWSPHN Mental Health Central Intake template into Best Practice and MedicalDirector, contact your Practice Support Officer.

 

For assistance
