HealthPathways – Up to date clinical information and local referral services

HealthPathways is an online clinical and referral information portal for health professionals. It improves care coordination between primary and secondary care services in South Western Sydney. 

 

HealthPathways is a shared project between South Western Sydney LHD and PHN

HealthPathways is a shared project between the South Western Sydney Local Health District (SWSLHD) and the South Western Sydney PHN (SWSPHN). 

This online clinical and referral information portal is used by clinicians at the point of care. The website is free to access for all health professionals practicing in South Western Sydney.

HealthPathways gives general practitioners access to management and treatment options for a range of clinical presentation and provides information about South Western Sydney clinical services and their referral processes.

Site users may customise the amount of detail displayed, making it easy to access during a consultation.

Links to reputable patient information and clinical resources are available in most HealthPathways. Information is translated where available.

There are currently more than 600 localised pathways on HealthPathways with another 150 in development.

Contribute to HealthPathways

Health professionals wanting to get involved in HealthPathways can do so in a variety of ways:

  • Attend workgroups to improve care coordination and the patient experience of medical care in South Western Sydney
  • Become a GP Clinical Editor and assist in the development of HealthPathways content
