HealthPathways is the clinical and referral information portal used at point-of-care by GPs who practice in South Western Sydney.
GPs access management and treatment options for a range of clinical presentations and obtain information about local clinical services and their referral processes. Each pathway provides clear and concise guidance for assessing and managing a patient with a particular symptom or condition. Pathways also include information about making requests to services in the local health system.
HealthPathways contains more than 700 localised pathways and continues to expand. Information is translated where available.
HealthPathways is free to access for all health professionals who practice in South Western Sydney.
Written and updated by local clinicians
HealthPathways is maintained by local health professionals and is a shared project between South Western Sydney Local Health District (SWSLHD) and South Western Sydney PHN (SWSPHN).
Content is developed collaboratively by general practitioners, hospital clinicians, and a wide range of other health professionals. Each pathway is evidence-informed and reflects local reality. It aims to preserve clinical autonomy and patient choice.

Information on HealthPathways is sourced from a range of health professionals who practice in South Western Sydney.
Participate in committee workgroups
Members of the HP steering committee, Health Alliance Fairfield, rsGP, and Diabetes Framework Committee contribute content on HealthPathways to improve care coordination and patient experience. GP working groups members are paid $150 per/hour through rsGP.
