HealthPathways is the clinical and referral information portal used at point-of-care by GPs who practice in South Western Sydney.
GPs access management and treatment options for a range of clinical presentations and obtain information about local clinical services and their referral processes. Each pathway provides clear and concise guidance for assessing and managing a patient with a particular symptom or condition. Pathways also include information about making requests to services in the local health system.
HealthPathways contains more than 600 localised pathways and continues to expand. Information is translated where available.
HealthPathways is free to access for all health professionals who practice in South Western Sydney.
Kept up-to-date by South Western Sydney health professionals
HealthPathways is maintained by local health professionals and is a shared project between South Western Sydney Local Health District (SWSLHD) and South Western Sydney PHN (SWSPHN).
Content is developed collaboratively by general practitioners, hospital clinicians, and a wide range of other health professionals. Each pathway is evidence-informed and reflects local reality. It aims to preserve clinical autonomy and patient choice.

Look for the HealthPathways password in your Practice Pulse emails. Practice Pulse is sent to all GPs who practice in South Western Sydney weekly. Alternatively, contact your PSO on 4632 3000 or email email@example.com.
