- Type 1 or gestational diabetes diagnosis
- Pregnant
- Active foot ulcer
- On renal dialysis
- Are seeking a license review
How to refer
Diabetes case conferences are available all general practice in South Western Sydney.
GPs can refer to this service by completing the Integrated Diabetes Care Case Conference Referral Form. Submit the completed form, along with the patient’s health summary and following pathology (done within the last 10 weeks):
- HbA1c
- Fasting lipids
- FBC
- UEC
- LFT
- Up-to-date urine ACR
You will then be contacted to arrange a time for the case conference.
Depending on the complexity of the patient, consultations generally take between 20-30 minutes.
