South Western Sydney PHN (SWSPHN) is committed to improve the end of life journey of people living in South Western Sydney. Health professionals have a strong role in initiating discussions regarding people’s wishes and expectations for their care as they become more ill, and also to advocate for their wishes when they reach a point where they are not able to take decisions for themselves.
