Clean Slate Clinic offers virtual care for clinically eligible clients struggling with dependence on alcohol, stimulants or cannabis to safely detox at home.
South Western Sydney PHN (SWSPHN) is funding a limited number of places for people in South Western Sydney who:
- could not otherwise afford the program
- or are from a culturally or linguistically diverse background
What to expect from the Clean Slate Clinic
Clean Slate Clinic provides an integrated program of assessment, medical withdrawal and recovery support for clients.
This includes:
- an initial comprehensive assessment by a nurse
- access to recovery groups
- doctor appointments to discuss recovery goals and relapse risks, and prescription of withdrawal medications
- liaison with local pharmacy
- detox support with daily nurse check-ins and clinical withdrawal assessment, and daily medication review and dispensing
- follow up appointments with a nurse for 12 months
Clean Slate Clinic will liaise with your regular GP (with your consent) to ensure they can contribute to your ongoing care coordination and management.
Clean Slate Clinic is a GP-friendly program
Clean Slate Clinic offers regular CPD webinars for healthcare providers on a range of AOD topics. With patient consent, Clean Slate Clinic will keep the GP informed at every step of their patient’s journey, offering recommendations for their ongoing team care and/or mental health support. GPs will receive a discharge summary which provides the information needed to prepare and bill a GP Management Plan and/or Mental Health Care Plan.

Clean Slate Clinic provides telehealth clinical support to help clients struggling with alcohol, stimulant or cannabis dependence to detox and recover safely at home.
Clean Slate Clinic also:
- works with other SWSPHN-funded services to provide assessment, withdrawal support and prescribing for individuals
- Is in partnership with SWSPHN and community organisations to increase accessibility to its services for culturally and linguistically diverse people, including awareness raising and tailoring of materials into priority languages
