The Mental Health Service Navigator supports people who are struggling with severe mental health challenges in South Western Sydney. This program connects the individual with essential local services, including:
- General Practitioners (GPs)
- Community support services, including housing, employment, education, family, and financial support
How the Mental Health Service Navigator assists clients
If you, a family member, or someone you care for is experiencing severe mental health issues, the Mental Health Service Navigator can provide tailored support. The Mental Health Service Navigator has extensive local knowledge of services available throughout South Western Sydney, and will help with:
- Referrals to medical, psychological, and social services
- Personal and confidential support
- Information and advice on a wide range of services
- Consultations available via phone, email, or in person
Supporting health professionals
The Mental Health Service Navigator actively works with health professionals, including GPs and Aboriginal Community Controlled Health Services (ACCHS), to inform them about the services available for managing severe mental health illnesses and related psychosocial disorders.
Eligibility criteria
This program delivers support to individuals with psychosocial disabilities, their carers, and families. Health professionals are welcome to use the service to help support their at-risk patients. People who are homeless or at risk of homelessness may also request support from the service navigator.
To meet program criteria, the individual:
- Will have a severe mental illness (may be undiagnosed) and associated psychosocial functional impairment.
- Had no restrictions in their ability to participate in the community fully and actively because of their residential setting (e.g. resides in prison or a psychiatric facility).
- Is not receiving similar psychosocial supports through a state or territory government program or the NDIS, where there is potential for duplication of service offerings.
- Is aged 16 years and over. (Exceptions for people younger than 16 years is subject to approval by South Western Sydney PHN.)
