SWSPHN 2025-2028 Needs Assessment

South Western Sydney PHN’s Needs Assessment 2025 – 2028 reflects a complete review and update of the previous report and the incorporation of recent consultations and literature review. The report identifies key areas SWSPHN has assessed as priority needs for the next three years.

The needs assessment is an ongoing process involving literature reviews, data collection and analysis, stakeholder and community surveys, consultations, and focus groups.

Antenatal Care Local Health Forum August /September 2025

Alcohol and other drugs local health forum Report April 2025

The area of focus of the forums held in April 2025 was Alcohol and Other Drugs in South Western Sydney, with an emphasis on improving health outcomes for priority populations in our region. The demand for drug and alcohol services has increased with population growth in South Western Sydney at a rate of approximately 10% per annum over the past decade.

CALD and Refugee communities co-design report 2024

This report outlines health and service needs for people who are from CALD and Refugee communities in SWS and lists recommendations to guide service commissioning.

SWSPHN Homelessness Co-design Report 2024

This report highlights the unmet health and service needs for people who are homeless or at risk of homelessness in South Western Sydney (SWS). It forms part of SWSPHN’s broader needs assessment and aligns with the Department of Health and Aged Care's PHN Homelessness Access Program.

Understanding end-of-life in South Western Sydney Local Health Forum Report 2024

SWSPHN understands the population of the entire region is growing and with an ageing population, demand for end-of-life services is rising. The purpose of this round of consultations was to better understand the enablers and barriers which impact end-of-life care in South Western Sydney and to find key priorities for our region.

Suicide Prevention Codesign report

This co-design was undertaken by South Western Sydney Primary Health Network (SWSPHN) in response to funding granted to each PHN in the 2022-2023 Federal Budget allocated for targeted regional initiatives in suicide prevention (TRISP). The findings and recommendations will inform SWSPHNs development of an action plan and commissioning decisions in relation to the program area.

Healthy ageing co-design report

Published April 2022.

The purpose of the Healthy Ageing Co-design was to identify the needs and opportunities to support older people to remain living in the community and delay their entry into residential aged care homes.
