Aim of meeting:
GPs will enhance their ability to recognise early signs of deterioration, undertake focused clinical assessments, and make appropriate escalation decisions. They will also strengthen skills in integrating advance care planning into acute management, initiating appropriate treatment, and communicating effectively with patients, families, and multidisciplinary teams to support safe, patient-centred care.
Learning outcomes:
- Identify and assess early clinical deterioration on elderly patients.
- Apply clinical judgement to determine appropriate escalation of care, incorporation goals of care and advance care planning.
- Implement and coordinate management plans and communicate clearly with care teams.
