CALD refugee and homeless healthcare service navigator.

Healthcare service navigation is now available for people from culturally and linguistically diverse (CALD) backgrounds, refugees and those experiencing, or at risk of, homelessness across South Western Sydney.

Service navigation helps individuals overcome challenges such as financial hardship, cultural and language barriers, and limited knowledge of available services. By improving access to healthcare, it makes navigating the healthcare system much easier.

 

What to expect

The Service Navigator provides a range of support to ensure individuals can access healthcare services more easily. Here’s what you can expect:

  • Easy support: Get help by phone, online, or in person.
  • Interpreter services: Language help is available for some appointments.
  • Care coordination: A navigator will guide you through the healthcare system and check on your progress.
  • Finding healthcare: Get help finding doctors or health services near you.
  • Medicare card: Assistance to apply for a Medicare card if you qualify.
  • Financial help: Support to pay for healthcare costs if needed.
  • Care plans: Plans made just for your health needs.
  • Health checks: Help with important health tests like cancer screenings.

By providing these supports, service navigators ensure individuals receive holistic care which is trauma-informed, culturally appropriate and focused on their needs.

The Service Navigator recognises the unique circumstances of every individual and tailors support accordingly.

  • For CALD and refugee communities: The navigator respects cultural differences and provides trauma-informed care designed to meet each person’s specific needs.
  • For people experiencing homelessness: Whether you are sleeping rough, couch surfing or living in temporary housing, the navigator offers compassionate and practical support to help you overcome healthcare barriers.

 

Who is this service for?

You may be eligible for this service if you:

  • come from a CALD or refugee background
  • are experiencing homelessness or at risk of becoming homeless
  • are a permanent resident or temporary visa holder living in South Western Sydney

You do not need to be an Australian citizen to use this service, but you do need to live in South Western Sydney, which includes the LGAs of Bankstown, Camden, Campbelltown, Fairfield, Liverpool, Wingecarribee and Wollondilly.
