Type 2 diabetes case conference – information for patients

Your GP may invite you to attend a type 2 diabetes case conference at their practice. With exclusive access to experts in diabetes, you will find better ways to care for yourself.

 

What to expect

A diabetes case conference takes 45-minutes. For the first half of your session a Diabetes Educator will give you advice to help look after your diabetes. Then your GP and an Endocrinologist will join in to make a plan for looking after your diabetes. An Endocrinologist is a doctor who specialises in hormonal conditions, including diabetes.

 

After the case conference

After your session, you will do a short survey. This survey measures your confidence to look after your diabetes and how you feel the case conference went. Your answers are kept private and only used to improve future services.

 

Follow-up appointment

You will be referred for a blood test five months after the case conference, your GP and the diabetes expert will review. Your GP will then discuss these results with you and suggest recommendations if needed.

 

Keeping a diary

You will need to keep a diary for four days before your session. Record your:

  • Blood sugar level
  • Food and drink
  • How much insulin you inject (if relevant)

Bring this diary with you to the case conference.

There are no extra costs to see the diabetes experts.
