GPs and Residential Aged Care Facility (RACF) clinical staff are encouraged to refer RACF residents experiencing rapid decline or acute changes in their condition to the Community Outreach Geriatric Service (COGS). The service delivered to residential aged care aims to maintain the health and independence of older people by providing rapid access to medical and nursing care while they remain in their RACF. In response to the COVID-19 pandemic, the COGS is providing a seven day a week service to all RACFs.
