Older people may have multiple morbidities and complex medication needs. Optimal medication management can be achieved using a systematic, multidisciplinary approach with the older person and or their representative.
Medication Management Reviews seek to identify, resolve and prevent medication related problems.
Reviews can occur in a person’s home or residential aged care facility with the MBS allocating item numbers dependent on location, 900 and 903 respectively.
An assessment for the need to refer for a medication management review is ideally undertaken as part of the annual 75+ Health Assessment.
