Connector Hub – psychosocial peer support for people with severe mental health challenges

Connector Hub provides free psychosocial peer support for people with severe mental illness not eligible for NDIS.

Connector Hub provides time limited recovery support for people living with severe mental health challenges to achieve their goals and improve wellbeing.

A recovery support worker and access worker can help you work on the goals you want to achieve.

You will be supported to take part in a range of activities to help you live your best life.

This program offers: 

• Social activities, outings and connecting people with their community
• Groups focusing on understanding their mental health challenges, improving physical health, and living the life they want 
• Individual support and assistance 
• NDIS application assistance
• Linking participants with services, supports and health professionals.
• Supporting participants to find work, education or accommodation, and to reach their education, employment and financial goals

Who can access Connector Hub?

Connector Hub can help people who:

• are aged 16+
• live with the challenges living with severe mental health challenges can cause in everyday life are not receiving funding under NDIS (National Disability Insurance Scheme)
• live in South Western Sydney local government areas of Bankstown, Camden, Campbelltown, Fairfield, Liverpool, Wingecarribee or Wollondilly

Accessing the Connector Hub program

Anyone can refer an eligible person to the Connector Hub (with their permission) including 

• self-referral
• carers, family or friends
• health professionals
