Podiatrists within South Western Sydney Local Health District (SWSLHD) have been working together to develop a new SWSLHD High Risk Foot Referral Form.
The form has been developed collaboratively to simplify the referral process for GPs and community podiatrists in our region.
Now clinicians can simply use the one referral form for all three High Risk Foot Clinics in our region.
The referral form clearly states that any patients referred needs to meet at least one of the following criteria:
- active foot wound
- gangrene/ necrosis or critical limb ischaemia
- suspected charcot arthropathy
- suspected or confirmed osteomyelitis or septic arthritis in the foot
The High Risk Foot podiatrists are currently working on specific actions deemed to be high priority by the South Western Sydney Diabetes Framework to 2026, a partnership between SWSLHD and SWSPHN.
High Risk Foot podiatrists are currently exploring ways to support the professional development of private podiatry services in the delivery of complex diabetic foot care.
They are also developing an early referral pathway between private podiatry services and high risk services, in collaboration with private podiatrists.
