About My Care Partners
My Care Partners is a program for practices to provide comprehensive, coordinated care to patients with chronic and complex conditions in South Western Sydney.
Co-designed by South Western Sydney PHN, South Western Sydney Local Health District and members of the community, My Care Partners adopts a ‘medical neighbourhood’ model of care.
Eligible practices joining My Care Partners become a patient-centred medical home, working with a dedicated care coordination service provider and other providers within the medical neighbourhood to support patients at risk of frequent hospitalisation.

My Care Partners: Our story so far
My Care Partners connects general practice with dedicated care coordination, community services and acute care to support patients with chronic and complex conditions who are at risk of frequent hospitalisation. Since the pilot in 2021, the program has grown and evolved.
How the program has grown
What the evaluation tells us
Early findings from Western Sydney University’s evaluation, show potentially preventable hospitalisation rates fell steadily, from 0.63 before enrolment to 0.58 during intervention and 0.54 during GP monitoring. The evaluation also found that following program participation, total health service costs were approximately $2,875 lower per patient than for a matched comparison group, driven primarily by reduced hospital admission costs.
100%
of surveyed practice staff would recommend My Care Partners to other GPs and practices
100%

92%

What patients say
Patients consistently report feeling listened to, involved in decisions about their care and well supported to achieve their goals.
‘Service and care I received was exceptional. Could not be happier with the help I have received.’
‘Thank you for listening and treating us with respect. We felt heard.’
