EOI - Primary Care Patient Reported Measures Working Group

On behalf of the NSW PHN CEO Group and the NSW Agency for Clinical Innovation, SWSPHN is a seeking a consumer representative to contribute to the Primary Care Patient Reported Measures (PRMs) Working Group.

Patient Reported Measures (PRM) give patients the opportunity to provide direct, timely feedback about their health-related experiences and outcomes. This feedback helps drive improvements in care across the health system. 

PRMs are broken into two categories: 

  • Patient Report Outcome Measures (PROM): Standardised, validated self-reporting instruments which use a patients’ views and opinions in order to assess their health status and overall well-being.
  • Patient Reported Experience Measures (PREM): Self-reporting instruments which measure a patients’ perception of their experience whilst receiving care.

The ACI has been leading the co-design, testing, implementation, of a state-wide data collection platform now known as HOPE (Health Outcomes and Patient Experience) since 2014, which is currently implemented across Local Health District and Specialty Health Networks.

The next iteration of HOPE will be piloted in up to 30 general practices across three PHNs this year. This will help inform how HOPE can meet the requirements for long-term use in primary care. We recognise the integral role of consumers in the health system, and it is essential we obtain input and advice from consumers as we progress this work.

The focus of the Primary Care PRM Working Group is to:

  • Review the data governance and management framework for HOPE to reflect the needs of PHNs and primary care to support the pilot of HOPE in up to 30 general practices.
  • Pursue a standardised long-term approach for collecting and reporting PRM across NSW/ACT PHNs, including investigating whether HOPE is fit for purpose for PHN and primary care use.

Working group meetings occur fortnightly on Fridays between 10am and 11am but may reduce in frequency overtime. Meetings are currently held virtually via Microsoft Teams. You will be remunerated for your time.
