Starting next month, general practices across South Western Sydney will be provided with a clearer picture of their participation in SWSPHN programs and services, and interaction with SWSPHN’s Service Support and Digital Partnerships teams.

SWSPHN will provide practices with a twice-yearly, individualised General Practice Engagement Report, between the months of January and June, and then again between July and December.

The report will be a visual summary of your practice with:

  • details of your interactions with SWSPHN, including the frequency of engagement, method of engagement and with which SWSPHN team you engage
  • the SWSPHN programs in which your practice currently participates
  • a snapshot of your staff numbers and the services you provide

The report will also provide you with:

  • recommendations on additional SWSPHN programs and services which could be of benefit to your practice. This includes an explanation of each program or service, links to additional information and who to contact to participate
  • recommendations are based on your practice profile and aim to support you in delivering the best care possible for your patients

The General Practice Engagement Reports are intended for use by your entire practice team, and will be a valuable tool in contributing to quality improvements within your practice.

Your SWSPHN Practice Support Officer will contact you in the coming weeks to organise an opportunity to discuss your report in detail, and provide support and guidance on how to participate in additional programs.
