SWSPHN has revamped our Health Resource Directory (HRD) to make it easier for you to provide patients with easy-to-read, reliable and locally-relevant information about their health condition and the services available to them.
HRD is a collection of health factsheets which have been formulated under the same strict clinical guidelines as our HealthPathways content.
You can access the HRD factsheets via HealthPathways.

The factsheets provide information about the condition, what patients can expect their GP to do next, the services available and how they can find more information.
You are encouraged to print the factsheets for patients to take away with them.
The original HRD website was launched in 2017. It has been updated to make it more mobile-friendly and accessible to people for whom English is not their first language.
The new site features 183 health conditions, 29 health procedures, 21 resources people can use to manage their health, and 27 pages of information about improving their overall health.
All of these have been published in English, Vietnamese, Simplified Chinese and Arabic, and will include audio versions in each language with more factsheets being added regularly.
