South Western Sydney Local Health District (SWSLHD) is seeking expressions of interest from South Western Sydney GPs to represent GPs on the Planned Care for Better Health Implementation Working Group.
The Planned Care for Better Health initiative is a Ministry of Health initiative which identifies patients at risk of hospitalisation early, and strengthens the care provided to them through Integrated Care interventions including care navigation and care coordination to improve their experience of receiving care and to keep them healthier over the long term.
As the GP representative, your commitment will be to attend fortnightly meetings via Skype. Your participation in the working group provides an opportunity to be involved in shaping implementation requirements to support the implementation of the Planned Care for Better Health initiative across SWSLHD.
You will be remunerated for your time.
