Help older people stay healthy, connected and independent

SWSPHN is seeking Expressions of Interest from organisations to deliver innovative programs to support older people to age well while living at home.

Funding is available for community-based initiatives that respond to local needs and deliver meaningful health and wellbeing outcomes for older people across South Western Sydney.

 

Funding available

Successful applicants may receive between $100,000 and $200,000 through a formal funding agreement.

  • total funding pool: $564,430
  • contract term of up to 12 months
  • opportunity for establishment and implementation phases where required

 

Priority areas

Proposed initiatives must align with one or more of the following priorities:

  • increase healthy lifestyle behaviours
  • reduce social isolation and loneliness
  • improve management of chronic disease
  • increase access to, and connection with, existing services

 

What we’re looking for

We are seeking innovative, evidence-informed and community-driven approaches that support older people to remain healthy, active and independent.

Initiatives may include workshops, education sessions, community activities, or targeted programs for priority populations.

Activities funded under this grants program should increase older people’s ability to age safely and independently at home, improve physical health, wellbeing and functional capacity through preventative activities and early intervention, reduce social isolation and loneliness and support healthy lifestyle behaviours.

An information session will be held on Monday, 29 June, at 2pm via Zoom.

The information session will include an overview of the EOI, outline the outcomes sought, the process for submission and to allow potential respondents to ask questions.

Intended outcomes

Successful initiatives will contribute to:

  • increased participation in community programs and activities
  • improved physical activity and healthy lifestyle behaviours
  • reduced social isolation and loneliness
  • improved awareness of local health and community services
  • greater confidence and ability to live independently at home

 

Who can apply?

Applications are welcome from:

  • community-based organisations
  • non-government organisations
  • government organisations
  • private providers
  • Aboriginal Community Controlled Organisations
  • partnerships and consortia are encouraged, with one lead organisation nominated as the contracting agency.

 

Target population

Programs must support:

  • older adults aged 65 years and over
  • Aboriginal and Torres Strait Islander people aged 50 years and over
