We know one in two Australians have at least one chronic disease and almost two in five adults have two or more chronic conditions. There are lots of things a person can do to avoid their risk of developing a chronic disease, such as eating a healthy diet, exercising regularly, avoiding smoking – but what we don’t know is what gaps exist in the treatment and management of chronic disease in South Western Sydney. 

SWSPHN hosted local health forums in Bowral, Campbelltown, Bankstown and Fairfield to provide community members the opportunity to share valuable feedback on the challenges and issues for people managing chronic disease.
