An innovative service for people living with a severe mental illness has been launched in South Western Sydney. South Western Sydney Primary Health Network (SWSPHN) has commissioned the Connector Hub South Western Sydney, a flexible new psychosocial service helping bridge the gap for people with a severe mental illness who are not eligible for the National Disability Insurance Scheme.
South Western Sydney PHN is a not-for-profit health organisation dedicated to supporting general practitioners, practice nurses and other primary health providers.
