iRAD Verbal Consent Granted template
GPs or other practice staff may opt to seek iRAD consent in writing or verbally.
If you choose to gain consent verbally, follow the steps and suggested script on this form when a patient has consented.
Save the template iRAD CONSENT GRANTED into documents in the patient's medical record. Name the record "iRAD Consent Granted".
iRAD will begin access from the moment this record is filed correctly.
iRAD Verbal Consent Denied template
GPs or other practice staff may opt to seek iRAD consent in writing or verbally.
If you choose to gain consent verbally, follow the steps and suggested script on this form when a patient has denied consent. Then save the template iRAD CONSENT DENIED into documents in the patient's medical record. Name the record "iRAD Consent Denied".
This will remove consent. No information will be recorded for this patient from your current care location in iRAD.
iRAD Written Consent Granted template
GPs and other practice staff may choose to gain iRAD consent verbally or in writing.
If you choose to gain consent in writing, ensure your patient reads and understands this form before they sign and date it. Then scan this document and save into documents in the patient's medical record. Name the record "iRAD Consent Granted".
iRAD will begin access from the moment this record is filed correctly.
iRAD Written Denied Consent template
GPs and other practice staff may choose to gain iRAD consent verbally or in writing.
If you choose to gain consent verbally, follow the steps and suggested script on this form when a patient has denied consent. Then save the template iRAD CONSENT DENIED into documents in the patient's medical record. Name the record "iRAD Consent Denied".
This will remove consent. No information will be recorded for this patient from your current care location in iRAD.
