A new service is available in South Western Sydney to help you manage your COVID-positive patients.

SWSPHN has commissioned the COVID-19 Monitoring Project which sees registered nurses (RNs) employed at participating ‘keystone’ practices to manage COVID-19 positive patients from across the region.

The project aims to alleviate the workload on GPs.

You are invited to refer your COVID-19 positive patients to the project to be managed by RNs for the duration of their diagnosis (for example, seven to 14 days).

You can refer your patients referred through a centralised intake process managed by SWSPHN.

Please note:

  • Only South Western Sydney residents can participate in the project
  • Patients should not exceed low-to-medium risk management
  • Patients will be monitored via a technology-based consultation process
  • GPs must gain patient consent to participate in the project before referral
