Residential aged care telehealth skills for GPs and nurses

A one-off incentive payment of $300 is available for each GP/GP Registrar practicing in South Western Sydney.

A one-off incentive payment of $160 is available for each nurse/RACF staff practicing in South Western Sydney.

Aim:
The activity is designed to equip aged care nurses and GPs with the appropriate skills and tools to leverage the telehealth equipment provided to the RACFs for the best health outcomes for the residents. 

Learning outcomes:

  • Utilise effective communication skills required to manage the aged care/GP interface in a telehealth environment.
  • Describe the ISBAR4AC communication protocol and its benefits in standardising communications.
  • Recognise and describe the deteriorating resident in an RACF and the triggers for escalation to medical consultation/care.
  • Discuss and identify the ‘Goals of Care’ in the management of the RACF resident.
