GP Guide for completing new PPE Order Form

Fact Sheet: Completing new PPE and Pulse Oximeter Order Form (GP, ACCHS and GPRCs).

This fact sheet provides GPs, ACCHS and GPRCs with guidance on completing the General Practice PPE and Pulse Oximeter Order Form, which you will receive through your PHN. The new process for PPE ordering, distribution and delivery which commenced on February 14, 2022, is illustrated in Appendix 1.

Living with COVID PPE Bundles and PPE Emergency Provision Order Form (GP, ACCHS and GPRCs)

This order form is intended to supply approximately one month’s supply of PPE per practitioner for:

• GPs, ACCHSs and GPRCs willing to manage COVID positive patients face-to-face, until 30 September 2022 (please order under Section 1)
• ACCHSs and rural and remote GPs willing to manage COVID positive patients virtually and respiratory patients face-to-face, where workforce capacity is limited AND the GP is serving the community for most or all health needs, until 30 September 2022 (please order under Section 1)
• GPs and GPRCs willing to support COVID positive people virtually, and respiratory patients face-to-face are eligible to access P2/N95 respirators and eye protection, until 30 September 2022 (please order under Section 1(a))

The following provides Allied Health professionals with guidance on completing the Living with COVID Allied Health PPE Order Form, which you will receive through your PHN. The process for PPE ordering, distribution and delivery is illustrated in Appendix 1.

Living with COVID Allied Health PPE Order Form

Emergency Provision PPE can only be accessed by Allied Health professionals where there is no local supply available commercially, or from another State or Territory government schemes.

Allied Health Professionals can access the following Emergency Provision PPE:

• Surgical masks - (All States) (1 box per practice)
• Surgical mask and Goggle / face shield - (QLD only, in line with Tranche 5 arrangements) (upto 2 units per practitioner - noting this item can be washed and re-used)
