A reminder to provide bank and employer account details to Medicare

Health professionals are reminded that if they’re applying for a Medicare provider number and have indicated they’ll be claiming a Medicare benefit, they need to provide the Department of Human Services with:

  • current bank account details 
  • details about employer or contracting organisation 

The quickest way to apply for a subsequent Medicare provider number is online through Health Professional Online Services (HPOS).

The application forms have been updated and simplified. You can now use the same form to register for an initial or subsequent provider location number. 

Important: From 9 September 2018, Medicare will return applications that don’t include bank and employer details.
